CEU Course Description

This course provides an overview of the opportunities for long-term and post-acute care providers to use health information technology to improve care delivery and health outcomes while reducing total cost of care.  This course: defines the issue; provides background information on the current landscape of health IT in LTPAC and an overview of the challenges and opportunities to improve care coordination and patient outcomes; highlights promising examples of how health IT tools and standards-based health information exchange are enabling patient-centered care; summarizes many health care transformation programs that represent opportunities for LTPAC providers to be integrated into health IT-enabled health care deliver and payment programs; and summarizes the key considerations for the LTPAC community today in deciding how to use health IT to transform care and provides suggestions for additional resources to aid the LTPAC community as providers implement tools and standards-based HIE.

CEU Course Objectives

  1. Discuss the areas included in the electronic health record functionality.
  2. Enumerate the data regularly submitted by patients using the local home health and visiting nurse organization tele-monitoring devices and describe how this is important to the health of high-risk diabetic patients.
  3. Explain the Longitudinal Coordination of Care Workgroup's primary goal.
  4. Investigate what the electronic registry for Physician Orders for Life Sustaining Treatment allows patients to do for their own health care decisions.
  5. Compare and contrast the different delivery models and programs available for long-term care.
