Diabetes is a serious, common, and costly disease that affects 25.8 million Americans. Health care professionals are beginning to use a team care approach to treating diabetes, which is greatly affecting patient outcomes and improving the lives of those suffering from the disease. This CEU provides nurses with information on how to implement collaborative, multidisciplinary team care for adults and children with diabetes in a variety of settings, while providing an understanding of how it can improve patients' quality-of-life and save lives. 

 

CEU Course Objectives

  1. Examine the social, economical and clinical impact of diabetes.
  2. Distinguish the differences between the Team Care Models and how they can affect diabetes patients.
  3. Evaluate the process of building and maintaining a diabetes care team, while obtaining an understanding of non-traditional team care approaches, care reimbursement and the impact of the health care providers participating in collaborative care practice.
